North Carolina General Statute 95-129 (1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)manufacturing area-- 1.A Budgit 1 ton overhead hoist was being used to lift plastic injection molds. The hoist had not been provided with an initial safety/operational inspection prior to being placed into operation. Reference:ANSI B30.16-1980; paragraph 16-2.1.1(a) Initial inspection. Prior to initial use, all new, altered or modified hoists shall be inspected by a designated person to ensure compliance with the applicable provisions of this standard. NOTE:Among other methods, one feasible and acceptable method to correct this hazard is to perform an initial inspection before continuing with use of this hoist. 2.A Budgit 1 ton overhead hoist had not receive a periodic inspection. Reference:ANSI B30.16-1980; paragraph 16-2.1.1(b)(2) Periodic inspection. Visual inspection by an appointed person who makes records of apparent external conditions to provide the basis for a continuing evaluation. NOTE:Among other methods, one feasible and acceptable method to correct this hazard is to perform periodic operational visual checks of the hoist. 3.A test report for a Budgit overhead hoist was not maintained. Reference:ANSI B30.16-1980; paragraph 16-2.1.5(b) Dated reports and records should be maintained at time and interval as specified in 16-2.1.1(b)(2). Records should be stored where they are available to appointed personnel. NOTE:Among other methods, one feasible and acceptable method to correct this hazard is to conduct a load test and maintain the test results in the work place available for observation.
