A. No back up alarm provided for dump trucks employer knew or should have

employees working near dump truck were exposed to the hazards of being
struck by truck
backing up without warning.  A feasible and useful method to correct this
hazard among
other methods is to repair back up alarm on dump truck at recycling public
works yard.
B. Employer knew or should have known that employees working near dump
truck were
exposed to the hazards of being struck by truck backing up without
warning.  A feasible and
useful method is to repair back up alarm on Mack Front Loader Truck #35,
located on public
works yard.
C. Employer knew oar should have known that employees working with chain
saws were
exposed to hazard of being struck by cutting chain when released.  A
feasible and useful
methods to correct this hazard, among other methods, is to repair chain
brake on one
Homelite and install chain brake on one Oregon chainsaw located in the
parks department
saw room.
