Hooks, Jacks, and Slings, ASME B30.10a-1996, paragraphs 10-1.2.1.2 and
10-12.1.3.
(B)Large Diameter Area:  On 1 June 2000, and at times prior thereto, the
Lodestar electric chain hoist was not inspected on a frequent and periodic
basis
in accordance with the American Society of Mechanical Engineers (ASME)
Safety Standards for Cableways, Cranes, Derricks, Hoists, Hooks, Jacks, and
Slings, ASME B30.16b-1995, paragraphs 16-2.2.1.2 and 16-2.2.1.3.
(C)Large Diameter Area:  On 1 June 2000, and at times prior thereto, the
overhead support structure for the Lodestar electric chain hoist was not
inspected on a frequent and periodic basis in accordance with the American

(E)Machine Shop:  On 1 June 2000, and at times prior thereto, the hook for

Duff Lynx, model #EMC500, 500 lb electric chain hoist was not inspected on
a frequent and periodic basis in accordance with the American Society of
Mechanical Engineers (ASME) Safety Standards for Cableways, Cranes,
Derricks, Hoists, Hooks, Jacks, and Slings, ASME B30.10a-1996, paragraphs
