employees were exposed to crushing injures from possible overhead
structure/hoist failure:
(A)Large Diameter Area:  On 1 June 2000, and at times prior thereto, the
hook
for the Lodestar electric chain hoist was not inspected on a frequent and
periodic basis in accordance with the American Society of Mechanical
Engineers (ASME) Safety Standards for Cableways, Cranes, Derricks, Hoists,
Hooks, Jacks, and Slings, ASME B30.10a-1996, paragraphs 10-1.2.1.2 and
10-12.1.3.
(B)Large Diameter Area:  On 1 June 2000, and at times prior thereto, the
Lodestar electric chain hoist was not inspected on a frequent and periodic
basis
in accordance with the American Society of Mechanical Engineers (ASME)
Safety Standards for Cableways, Cranes, Derricks, Hoists, Hooks, Jacks, and
Slings, ASME B30.16b-1995, paragraphs 16-2.2.1.2 and 16-2.2.1.3.
(C)Large Diameter Area:  On 1 June 2000, and at times prior thereto, the
overhead support structure for the Lodestar electric chain hoist was not
inspected on a frequent and periodic basis in accordance with the American
Society of Mechanical Engineers (ASME) Safety Standards for Overhead and
Gantry Cranes (Top Running Bridge, Single Girder, Underhung Hoist), ASME
B30.17-1992, paragraphs 17-2.1.2 and 17-2.1.3.
(D)Large Diameter Area:  On 1 June 2000, and at times prior thereto, the
supporting structure and the Lodestar electric chain hoist did not have a
record
documenting that an appropriate rated load test had been performed, in
accordance with the  America Society of Mechanical Engineers (ASME) Safety
Standards for Overhead and Gantry Cranes (Top Running Bridge, Single
Girder, Underhung Hoist), ASME B30.17-1992, paragraphs 17-2.2.2.
(E)Machine Shop:  On 1 June 2000, and at times prior thereto, the hook for
the
Duff Lynx, model #EMC500, 500 lb electric chain hoist was not inspected on
a frequent and periodic basis in accordance with the American Society of
Mechanical Engineers (ASME) Safety Standards for Cableways, Cranes,
Derricks, Hoists, Hooks, Jacks, and Slings, ASME B30.10a-1996, paragraphs
10-1.2.1.2 and 10-12.1.3.
(F)Machine Shop:  On 1 June 2000, and at times prior thereto, the Duff
Lynx,
model #EMC500, 500 lb electric chain hoist was not inspected on a frequent
and periodic basis in accordance with the American Society of Mechanical
Engineers (ASME) Safety Standards for Cableways, Cranes, Derricks, Hoists,
Hooks, Jacks, and Slings, ASME B30.16b-1995, paragraphs 16-2.2.1.2 and
16-2.2.1.3.(G)Machine Shop:  On 1 June 2000, and at times prior thereto,
the Duff Lynx,

10-12.1.3.
(J)Machine Shop:  On 1 June 2000, and at times prior thereto, Coffing 500
lb
electric chain hoist was not inspected on a frequent and periodic basis in
accordance with the American Society of Mechanical Engineers (ASME)
Safety Standards for Cableways, Cranes, Derricks, Hoists, Hooks, Jacks, and
Slings, ASME B30.16b-1995, paragraphs 16-2.2.1.2 and 16-2.2.1.3.
