safe and healthful
for employees, and did not do every other thing reasonably necessary to
protect the life, safety,
and health of the employees:
a)  While making x-rays of welds on an existing gas main, near the
intersection of Fern Ridge
road and Highway 22, Stayton Oregon, an employee was in a excavation that
was not shored,
sloped or benched, as required by 29 CFR 1926.652(a)(2), to protect the
employee from
potential cave in.
