Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of falling down a freight elevator hoistway:
a)First Floor Freight Elevator Landing:  A string was attached to the
mechanical
locking mechanism of the hoistway vertical gate, allowing employees to
open the
hoistway gate from the landing side without the elevator car being in the
landing
zone and exposing employees to the hazard of falling into the hoistway, on
or
about 02/02/00.
Abatement Note:  One feasible and acceptable abatement method, among
others to correct this
hazard, is to remove the string from the hoistway vertical gate mechanical
locking mechanism
and locate the mechanical locking mechanism so that it can not be accessed
from
the landing side
when the hoistway gate is closed as per ANSI A17.1 - 1971, Section 111,
Rule 111.4d.
