North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were being exposed to a 20' fall hazard while working and riding in a debris box/personnel basket that did not meet the ASME B56.6-1992, part III-8.25.1 requirements. a)site - Employees re-shingling roof were working from and riding up in a 8'5"x3'x2' debris box/personnel basket attached to the forks of a John-Deere model #SS636 Rough Terrain Forklift. The debris box/personnel basket did not meet the requirements of ASME B56.6-1992. Employees were exposed to a 20' fall onto a concrete sidewalk/asphalt parking lot. (Ref. ASME B56.6-1992, Part III-8.25.1) one feasible and acceptable method, among others, to correct the hazard is to replace the debris box/personnel basket with one that meets the requirements of ASME B56.6-1992,part III-8.25.1.part
