North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: hazards associated with falling off a "job made" fixed wooden ladder (reference: ANSI A14.4-1979; 4.5.2): a)Sexton's office- where an approximately 13 foot "job made" wooden ladder was not equipped with filler blocks between the rungs. One feasible acceptable abatement method to correct this hazard is to remove this job made ladder and purchase a portable extension ladder to be used as needed.
