Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that:
a) Wheaton USA, Inc., Algroup Wheaton, Des Plaines, Illinois - employees
were exposed to
fire and explosion hazards where plastic food grade hose was used as gas
hose connectors to
supply natural gas to "flammers" on the automatic conveyor lines of
injection blow molding
machines used throughout the plant.  The hose was non-certified for use
with natural gas.
Among other methods, one feasible and acceptable method to correct this
hazard
is to comply
with National Fire Protection Association (NFPA) 54, National Fuel Gas
Code, Section 5.5.2,
which requires only listed gas hose connectors of minimum length not to
exceed 6 feet, be used
in accordance with their listing information.
Abatement documentation is required for this item in accordance with the
requirements of 29
CFR 1903.19(d).s

employees in that:
a) Wheaton USA, Inc., Algroup Wheaton, Des Plaines, Illinois - employees
were exposed to
the hazard of being burnt by molten plastic that was purged without the
use of interlocked safety
gates or guards on machinery that included, but was not limited to #111,
#112, #113, #114,
#115, #116, #117, #118, #119, #212, #611, #612, #717.  The molten plastic
is
purged at
temperatures ranging from 300-520 degrees.
Among other methods, one feasible and acceptable method to correct this
hazard is to comply
with American National Standards Institute (ANSI) B151.21 Standard for
Injection Blow
Molding Machines - Safety Requirements for Construction Care and Use,
Section 6.3.4, which
requires employees/operators be protected from hot plastic during purging
of the machines by
the use of interlocked safety gates or guards that will prevent purging
when the gates/guards are
not in place.  In addition to the use of interlocked gates/guards,
personal protective equipment
is to be provided and its use shall be strictly enforced by the employer.
Or in the alternative:
29 CFR 1910.212(a)(1): Machine guarding was not provided to protect
operator(s) and other
employees from the hazard(s) created by flying chips and sparks:
a) Wheaton USA, Inc., Algroup Wheaton, Des Plaines, Illinois - employees
were exposed to
the hazard of being burnt by molten plastic that was purged without the
use of adequate safety
gates or guards on machines including, but not limited to #111, #112,
#113, #114, #115, #116,
#117, #118, #119, #212, #611, #612, #717.  The molten plastic is purged at
temperatures
ranging from 350 - 520 degrees.
Abatement documentation is required for this item in accordance with the
requirements of 29
CFR 1903.19(d).s

the hazard of being caught in, struck by, and burnt, while required to
operate injection blow
molding machinery without training and instruction in the safe methods of
operation of injection
blow molding machine safety devices.
b) Wheaton USA, Inc., Algroup Wheaton, Des Plaines, Illinois - The
employer did not provide
adequate supervision to ensure correct operating procedures of injection
blow molding machinery
were being followed.
Among other methods, one feasible and acceptable method of correcting this
hazard is to comply
with American National Standards Institute (ANSI) B151.21 Standard for
Injection Blow
Molding Machines - Safety Requirements for Construction, Care and Use,
Section 7.1, which
requires the employer to train and instruct personnel in the safe method
of operation of the safety
devices before starting work on any operation covered in the ANSI
Standard.  In addition to
proper training the employer shall ensure by adequate supervision that
correct operating
procedures are being followed.
Abatement documentation is required for this item in accordance with the
requirements of 29
CFR 1903.19(d).

One feasible and acceptable method of correcting this hazard is to comply

product manufacturers instructions for the application and use of
interlock devices.
Abatement documentation is required for this item in accordance with the
requirements of 29
CFR 1903.19(d).
