recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to injuries resulting from the unexpected  failure
of a  battery lifting
attachment and subsequent release of the suspended load :
a) BATTERY MAINTENANCE AREA; THE BATTERY  LIFTING ATTACHMENT
USED  WITH  A FORK TRUCK TO LIFT 1500 POUND LEAD ACID BATTERIES
FROM ELECTRIC PALLET  JACKS HAD NUMEROUS  RECOGNIZED HAZARDS
INCREASING  THE LIKELIHOOD OF FAILURE. SPECIFICALLY, THIS LIFTING
ATTACHMENT DID NOT HAVE A LOAD RATING, WAS NOT SECURED TO THE
TRUCKS FORKS TO PREVENT THE ATTACHMENT FROM SLIDING OFF THE
FORKS DURING TRANSPORT, AND HAD DEFECTS SUCH AS SPLITTING OF THE
WOOD BEAM, EXCESSIVE WEAR,  AND DAMAGE OF THE ATTACHMENT HOOK
SLOTS DUE TO EITHER POOR DESIGN OR SUBSTITUTION OF ORIGINAL PARTS
WITH UNAPPROVED COMPONENTS.
