Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employerdid not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a. Building 2 and Flake Screen Building - On or about February 8, 2001, the elevator doors on the second floor and/or first floor (A unit, C and D unit and Flake Screen Unit) did not have an operative interlocking device that would prevent access to elevator shaft when elevator car was in use and/or at the first floor level. Employees were thereby exposed to hazards associated with shaft opening. A feasible method of correcting the hazard is to conform to the specifications referenced in "ANSI/ASME A17.1 Section 204.4/1993.
