Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following conditions: (a)Well Tech Engineering Services dba RTI Energy Systems, 7211 Spring Cypress, Spring, TX, 77379, shop made hook used in Pipe Shop on jib crane Appl# P01 FMJ type to support pipe connectors after tack welding exposing employee(s) to the hazard of a failing hook resulting in a falling connector. One feasible and useful abatement methods to correct this hazard, include, but are not limited to: remove the shop made hook and attach the synthetic web sling directly to the jib crane hook or test and inspect the lifting device under the direction of a qualified person with a written report furnished by such person confirming the load rating of the lifter as outlined in ANSI Standard B30.20 - Below-the-Hook Lifting Devices, section 20-1.4.2. *** Certification and Documentation of Abatement Required ***
