Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to being caught in moving conveyor parts:
a)Tire Facility, On or about 3/26/02:  The conveyor system was not provided
with any start up alarms.
Among other methods, one feasible method of abatement is to:
1) Provide an audible alarm for all areas of the conveyor as required per
ANSI B20.1-1976, section 5.11.2.2.6,
