Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did
not furnish employment and a place of employment that were free from
recognized
hazards that were causing or likely to cause death or serious physical
harm to employees
in that an employee was exposed to THE HAZARD OF FALLING:
a)West wing of building, on or about 11/6/02:  a manual boatswain's chair
was used in a window cleaning operation at a height of approximately 90
feet.
ABATEMENT NOTES:
Ensure that manual boatswain's chairs are not used where the height of
suspension exceeds 75 feet per American National Standards Institute,
Safety Requirements For Window Cleaning, ASME/ANSI A39.1-1987,
paragraph 6.1.2.
b)West wind of building, on or about 11/6/02: Emergency decent
equipment(equipment which is operational in the down direction only) was
used for a window cleaning operation.
ABATEMENT NOTES:
Ensure that emergency decent equipment is not used for window cleaning
operations per American National Standards Institute, Safety
Requirements For Window Cleaning, ASME/ANSI A39.1-1987, paragraph
11.3.
SW Cleaning Inc. was previously cited for a violation of this Occupational
Safety and Health Standard or its equivalent standard, Section 5(a)(1),
which was contained in OSHA Inspection Number 107693376, Citation
number 1, Item number 1, issued on 6/22/2000, with respect to a
workplace located at 500 Plum Street, Syracuse, New York 13204.
