Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a fall hazard while being lifted on the front forks of a
forklift to a height of
approximately 12 feet, on or about April 30 2002.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS:
1.  Employer shall comply with the requirements for lifting of personnel
on platforms mounted
on the forks of powered industrial trucks as referenced in OSHA CFR
1910.178 and ASME
B56.1 - Safety Standard for Low-Lift and High-Lift Trucks.
