Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer does not
always ensure to furnish employment and a place of employment which were
free from
recognized hazards that were causing or likely to cause death or serious
physical harm:
On or about June 18, 2003, at approximately 6:45 am, CSHOs witnessed an
explosion at the
north end melt deck.  The root cause was established to be water in the #2
furnace pit.  Molten
metal spilled from the ladle striking the water in the pit and causing a
blowout. Employees
continued to pour the heat without knowing the cause of the explosion
which could have exposed
employees to another explosion and possible injury or death.
Instance (1):  Employees were exposed to the recognized hazard of
water/molten metal
explosions without an emergency action plan in case of an explosion.
A feasible and acceptable abatement method to correct this hazard would
be:  Develop and
implement an emergency action plan addressing explosions/blowouts.
Instance (2):  Employees were exposed to the recognized hazard of
water/molten metal
explosions resulting from:
a)  Inadequate procedures for cleaning the pit when water is detected to
ensure
completely dry conditions.
b)  Inadequate procedures to inspect the pit to ensure no water was
present.
c)  Communication barrier between english speaking and non-english speaking
employees on the melt deck.
d)  Inadequate procedures to define the minimum number of employees
required
for a pour.
A feasible and acceptable abatement method to correct this hazard would
be: Develop and
implement procedures to ensure the pit is inspected and completely dry
especially after a leak,
develop and implement procedures specifying minimum requirements for the
operations, ensure
communication between all members of the melt and pour crews is
unquestionable.

this violation
has been abated within 10 calendar days after the abatement date.***t
