Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and/or falling hazards: (a) Gordon Signs Inc. at 544 S. Colorado, Denver, CO 80222: On or before March 8, 2002, the employer did not install an anti-two block device on its Skyhook crane, serial # 115130320568, Truck #46, thereby exposing an employee to crushing and/or falling hazards. ANSI Standard A10.28, Section 3.4 requires telescoping cranes used for work platform service to have two-blocking damage prevention features. Abatement Note: Abatement certification and documentation are required for this item (See enclosed "Sample Abatement-Certification Letter"). Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to have the crane manufacturer install anti-two block devices on the Skyhook brand crane.
