Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which was free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were working in a self propelled extendible boom aerial lift and were not utilizing an appropriate fall arrest system. a)4500 Belmont Road jobsite, northwest corner - Employees working in the Terex extendable boom aerial lift #1643 approximately 30 feet above grade, were exposed to fall hazards. Employees were not utilizing a fall arrest system. b)4500 Belmont Road jobsite, northwest corner - Employee working in the Terex extendable boom aerial lift #1643 approximately 30 feet above grade, was exposed to fall hazards. Employee was standing on the guardrails to gain additional height. c)4500 Belmont Road jobsite, southwest corner - Employees using the Terex extendable boom aerial lift #1845 for access/egress to the decking platform approximately 32 feet above grade, were exposed to fall hazards. Employees were not utilizing a fall arrest system. Among feasible means of abatement would be to comply with the American National Standards Institute, ANSI A92.5-1992 Standard for Boom-Supported Elevating Work Platforms. NO DOCUMENTATION OR CERTIFICATION REQUIRED FOR THIS ITEM
