Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to impact hazards from press parts and fasteners being ejected from the hydraulic press: The employer has a 900psi vertical hydraulic power press identified as CN09. The stripper plate and pipe die positioning plate were temporarily held together with two adjustable tool clamps and C-clamps. The loosening or the failure of the temporary adjustable fasteners may allow misalignment during operation causing parts to be ejected. Among other methods, feasible and acceptable abatement to correct this hazard through design (ANSI B11.2-1995) are replacing tool/c-clamps with die clamps or attaching the plates to the press with bolts.
