Title 40.1-51.1.(a); Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing, or were likely to cause, death or serious physical harm to employee(s) in that: a.) pivot restraints, required by ANSI/ALI B153.1-1990, Section 4.1.5, American National Standard for Automotive Lifts-Safety Requirements for the Construction Care and Use, a nationally recognized consensus document, were removed from the swing arms of 3 Model #s SP88, SP088, SP0A88 or variant of Rotary brand, two-post, surface mount automotive lifts manufactured by Dover Corporation and in use in the main repair bay area at 22510 S. Sterling Blvd, Sterling, VA. Employee(s) exposed to a struck-by or caught-between hazard. b.) the owner or employer could not document that he had established a periodic inspection procedure as recommended by the manufacturer to ensure the safe operation of Rotary brand, two-post, surface mount automotive lifts manufactured by Dover Corporation and in use in the main repair bay area at 22510 S. Sterling Blvd, Sterling, VA. as specified in ANSI/ALI B153.1-1990 Section 7.3. Employee(s) exposed to a struck-by or caught-between hazard. Note: One feasible and acceptable means to correct this hazard includes training employees in the safe use and operation of such equipment and establishing a routine preventive maintenance and inspection schedule with a commercial automobile lift service company.
