Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the recognized hazard of being struck by falling portions of the hoisted load(s) on the following overhead hoists: a) The manual overhead hoist of unknown capacity and manufacture located in the welding shop and another located at the west end of the road barrier manufacturing area did not have a latches on their load hooks, the overhead structures on which the hoists were mounted had not been load rated by the employer to determine the load capacity, the hoists were not labeled as to rating, manufacturer or operating warnings/guidelines and the hoists had not been inspected daily to ensure all safety devices were functioning appropriately. The hoists were utilized by employees to lift tools and materials and these deficiencies did potentially expose them to the hazards of an overloaded hoist and/or a dropped load. Among other methods, one feasible and acceptable abatement method to correct this hazard is to provide an undamaged load hook - with a latch, in accordance with Paragraph 16-1.1.2.4(d) of the American National Standards Institute (ANSI) B30.16- 1973, Overhead Hoists. Among other methods, one feasible and acceptable abatement method to correct this hazard is to determine the load rating capacity of the overhead structure on which the hoist is mounted, in accordance with Paragraph 16-1.1.3.1 of the American National Standards Institute (ANSI) B30.16-1973, Overhead Hoists. Among other methods, one feasible and acceptable abatement method to correct this hazard is to perform frequent inspections (daily) of all safety devices to determine malfunctions, in accordance with Paragraph 16-1.2.1.1(b)(1) of the American National Standards Institute (ANSI) B30.16-1973, Overhead Hoists. Among other methods, one feasible and acceptable abatement method to correct this hazard is to maintain the rating of the hoist permanently on the hoist or load block, in accordance with Paragraph 16-1.1.1.1 of the American National Standards Institute (ANSI) B30.16-1973, Overhead Hoists. Among other methods, one feasible and acceptable abatement method to correct this hazard is to maintain the manufacturer's name, address, and unit identification of the hoist permanently, in accordance with Paragraph 16-1.1.1.2 of the American National Standards Institute (ANSI) B30.16-1973, Overhead Hoists. Among other methods, one feasible and acceptable abatement method to correct this hazard is to maintain the WARNING legend and cautionary operating language on the hoist, in accordance with Paragraph 16-1.1.1.3 of the American National Standards Institute (ANSI) B30.16-1973, Overhead Hoists. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by unsecured air hose connections. a) Employees were observed using two air hoses that were not secured together with retainer clips or lanyards. These hoses were used to provide air that was being used to clean the concrete forms in the casting shed. Employees were exposed to a struck-by hazard. Among other methods, one feasible and acceptable method for abatement is to properly secure the hose couplings with safety clips, retainers or other positive means to prevent the couplings from becoming disconnected in accordance with manufacturer recommendations. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of overloading of a hydraulic boom crane or to the hazards of a dropped load due to the lack of a load rating chart: a) Employees were observed operating the Dressor Hydraulic Boom Crane and there was not a load rating chart available in the cab of the crane. This did expose employees to the hazards of overloading the crane and dropped loads. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure that there is a load rating chart with clearly legible and figures securely fixed to the crane cab and easily visible to the operator when seated at the controls in accordance with Section 15-1.1.3 of the American National Standards Institute (ANSI) B30.15-1973, Mobile Hydraulic Cranes. This did expose employees to the hazards of overloading the cranes and dropped loads. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure that the Mobile Hydraulic Cranes receive Periodic and Frequent inspections at the required intervals in accordance with Section 15-2.1.1; Inspections of the Hydraulic Components in accordance with Section 15-2.1.4; and inspection records are maintained in accordance with Section 15-2.1.6 of the American National Standards Institute (ANSI) B30.15-1973, Mobile Hydraulic Cranes.
