Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being injured by fire or explosion: (a) Location - Refrigeration room: On or about 6/25/03, employees were exposed to fire and explosion hazards when a container of Di-(4-tert-butylcyclohexyl) peroxydicarbonate, trade name Norox 600, was exposed to a piece of hot metal. (b) Location - Refrigeration room: On or about 6/25/03, employees were exposed to fire and explosion hazards when a container of Di-(4-tert-butylcyclohexyl) peroxydicarbonate, trade name Norox 600, was exposed to phenolic resin, a reducing agent. (c) Location - Refrigeration room: On or about 6/25/03, employees were exposed to fire and explosion hazards when Di-(4- tert-butylcyclohexyl) peroxydicarbonate, brand name Norox 600, was stored inside the refrigeration room, when there were no provisions for appropriate handling during a power outage on a hot day. (d) Location - Refrigeration room: On or about 6/25/03, employees were exposed to fire and explosion hazards when hexaneperoxoic acid, 2-ethyl,1,1-dimethyl-ethyl ester, brand name Trigonox 21-OP50, were stored inside the refrigeration room, when there were no provisions for the appropriate handling during a power outage on a hot day. Feasible and acceptable abatements include but are not limited to: 1. Prevent the handling of hot metal near flammable or combustible materials. 2. Segregate storage of organic peroxides from other incompatible materials. 3. Provide an emergency power source for the refrigeration room.
