employee, conditions of
employment and a place of employment free from recognized hazards which
caused or were
likely to cause death or serious injury to employees:
The employer did not ensure that employees were protected from fall
hazards of
approximately 22 to 23 feet while being lifted and transported by a
forklift truck,
working from an elevated work platform that did not have a complete
guardrail system.
The employees were not using personal fall protection equipment.
