Bridgestone Firestone Corporation
Instance A:
1. The employer did not ensure operator training on automotive lifts.
2. Operating instructions for automotive lifts were not displayed in a
conspicuous location.
3. Operator training was not documented.
4. Recommended inspections procedures of the lift manufacturer were not
followed.
5. The continued use of defective automotive lifts was allowed before
appropriates repairs were
made.
6. Periodic inspection procedures in accordance with the lift manufacturer
were not established.
7. Automotive lift inspector did not meet the necessary qualifications.

North wall.
20. Hydraulic piston on lift located along the South wall was leaking and
drifting
when
extended.
21. Lift locks were not operational on the following lifts numbered from
the North wall - Nos.
1, 3, 5 and 8. Lift No. 2 was equipped with manual locks that were not
inserted when the lift
was in the up position.

Automotive lift
Guidelines (AL ALOIM) - 1994 or ANSI B153.1 - 1974 or B153.1 - 1990
