Each employer shall furnish to each of his employees employment and a
place of employment
which are free from recognized hazards that are causing or are likely to
cause death or serious
physical harm to his employees while operating unsafe hydraulic and
mechanical
vehicle lifts.
Bridgestone Firestone Corporation
Instance A:
1. The employer did not ensure operator training on automotive lifts.
2. Operating instructions for automotive lifts were not displayed in a
conspicuous location.
3. Operator training was not documented.
4. Recommended inspections procedures of the lift manufacturer were not
followed.
5. The continued use of defective automotive lifts was allowed before
appropriates repairs were
made.
6. Periodic inspection procedures in accordance with the lift manufacturer
were not established.
7. Automotive lift inspector did not meet the necessary qualifications.
8. A record of each periodic inspection was not prepared or maintained.
9. Inspections by the employer for automotive lifts did not meet the
recommended manufacturer
guidelines as to frequency.
10. The lift inspector did not follow the inspection guidelines
established by the manufacturer.
11. The employer did not establish a periodic preventive procedure in
accordance with the
recommendations of the lift manufacturer.
12. Automotive lift preventive maintenance technician did not meet the
qualifications established
under the Automotive Lift Institute ALOIN-1994.
13. Formal training was not provided for preventive maintenance
technicians.
14. A thorough record of each preventive maintenance procedure was not
prepared
and
maintained.
15. Preventive maintenance did not follow the recommendations of the lift
manufacturer as to
frequency.
16. The employer did not perform repair maintenance procedures as noted by
lift operators.
17. A thorough record of each repair maintenance procedure was not
maintained.
18. Repair maintenance did not follow the recommendations of the lift
manufacturer and did not
meet the timely response to observations or recommendations by the lift
operator or qualified
lift inspector or preventive maintenance technician.
19. Chocking system was removed from from the runway on a mechanical lift
6th from the
North wall.
20. Hydraulic piston on lift located along the South wall was leaking and
drifting
when
extended.
21. Lift locks were not operational on the following lifts numbered from
the North wall - Nos.
1, 3, 5 and 8. Lift No. 2 was equipped with manual locks that were not
inserted when the lift
was in the up position.
Among feasible means of abatement the employer can comply with the
Automotive lift
Guidelines (AL ALOIM) - 1994 or ANSI B153.1 - 1974 or B153.1 - 1990
