Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to burns by hot water that was released during the
disconnecting of the piping
which carried the water:
a- Building C Warehouse: Written procedures were not followed and enforced
to ensure
that employees performing maintenance services do not work on the wrong
pipe.
One feasible and acceptable method to abatement this condition is to
follow procedures
as follows:
1- Train employees and re-enforce the lockout/tagout procedures to ensure
that
employee(s) do not lock/tag out the wrong pipe during maintenance
operations.
2- Obtain a line breaking permit before starting work which would have
confirmed that the wrong line was locked/tagged out by a Lead Mechanic or
Mechanic Foreman.
Violation observed on or about 6/17/03.
