Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing injuries due to unintentional operation of the
unguarded foot pedal on
the mechanical iron worker:
(a)Railing Shop, Forrest Steel, Pittsburgh, PA:  On or about 12-5-03 the
foot pedal used
to activate the punch portion of the Kling Bros. Engineering Works iron
worker, serial #3596
was not guarded to prevent accidental contact.  The foot pedal will
continue to cycle the ram
when pressure is applied.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to install a guard on the foot pedal and to install single stroke
operation
devices.
