Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to struck-by and/or caught-in between  hazards, in that:
(a)On or about February 9, 2004, the 2000 Ford F250 Pickup (VIN:
1FTNF21FOYEA25305) was modified with a deicing salt tank that obstructed
vision to the rear when backing, exposing workers in the lot to a struck by
hazard.
Among other methods, feasible and acceptable abatement methods to correct
this
hazard include equipping such vehicles with a reverse signal alarm audible
above
the surrounding noise level, additional rear view mirrors, or use of signal
personnel during backing.
Certification, and documentation/evidence of abatement is required.
Certification of abatement is required by 29 CFR Part 1903 and is to be
submitted
within ten (10)
calendar days after the abatement date.ment
