Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to crushing hazards in that:
a.Employees of Material Handling Equipment Erector's, Inc., working in the
GM warehouse, remained on the platform, attached to a Komatsu Model
number FT25T powered industrial truck, while it was in the raised position
and moved for more than minor adjustments during horizontal positioning, as
determined on 10/18/05.
NOTE: Among other methods, one feasible and acceptable abatement method to
correct this hazard is to comply with the requirements of ASME 56.1-2000,
Safety
Standard for Low Lift and High Lift Trucks, paragraph 4.17.3(l).
b.Employees of Material Handling Equipment Erector's, Inc., working in the
GM warehouse, were using a work platform, attached to a Komatsu Model
FT25T powered industrial truck, that did not have the following information
indicated on the platform:  maximum load including personnel and equipment,
weight of empty platform, and minimum capacity of truck on which the
platform can be used, as determined on 10/18/05.
NOTE:  Among other methods, one feasible and acceptable abatement method to
correct this hazard is to use a work platform that is specifically
designed for use on
fork lifts and meets the requirements of ASME 56.1-2000, Safety Standard
for Low
Lift and High Lift Trucks, paragraph 7.36.3(i).
