SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards which may cause death or serious physical harm to his
employees and comply with this regulation and other occupational safety and
health rules and regulations promulgated under Chapter 15 of Title 41,
Code of
Laws, State of South Carolina, 1976, as amended, as follows:
A.  Employer knew or should have known that the operator of a JCB 506C
Model
forklift, Vehicle Identification Number SLP506C02E0587237, not at the
controls
while one employee was elevated in a work basket, in excess of 20 feet
high at
south roof area at east side of building, constituted a hazard of the lift
shifting, or of the employee falling, or of the lift being struck by
objects
or other hazards and no way for the employee to get down.  A feasible and
useful method to correct this hazard, among others, is to have the operator
remain at the controls while the employee is in an elevated basket.
B.  Employer knew or should have known that the operator of a Gehl 553
Model
forklift, Serial Number JMO43492, not at the controls while one employee
was
elevated in a work basket, in excess of 20 feet high at south roof area at
west side of building, constituted a hazard of the lift shifting, or of the
employee falling, or of the lift being struck by objects, or other hazards
and
no way for the employee to get down.  A feasible and useful method to
correct
this hazard, among others, is to have the operator remain at the controls
while the employee is in an elevated basket.
