Carolina:  The employer did not furnish each of his employees conditions
of employment and a
place of employment free from recognized hazards that were causing or
likely to cause death or
serious physical harm to employees in that employees were exposed to a
struck by hazard:
a)facility - where a floor-operated underhung crane was being used to move
large
radiators (up to 400-600 pounds) and the load ratings of the crane bridge
and
runways were not marked.  REF: ASME B30.11-1998, Monorails and Underhung
Cranes.
b)facility - where a CM Valustar electric hoist (1 ton) was used to lift
large radiators
(up to 400-600 pounds) approximately 4 - 41/2 feet above the floor and a
label
displaying information concerning operating procedures was affixed to the
hoist,
load block, or controls.  REF: ASME B30.16-1998, Overhead Hoists
(Underhung).
c)facility - where the hook supplied with the CM Valustar electric hoist
did not have
a latch to bridge the throat opening of the hook and the manufacturer's
identification was not visible on the hook.  REF: ASME B30.16-1998,

d)facility - where a mechanical lifting device was being used to lift
large radiators
(up to 400-600 pounds) and the device was not marked with the rated load
and
test reports for a rated load test were not available.  REF: ASME

Below-The-Hook Lifting Devices.
ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS
HAZARD IS TO ENSURE THAT FREQUENT AND PERIODIC INSPECTIONS ARE
CONDUCTED ON THE CRANE, HOIST, HOOK, AND LIFTING DEVICE.  ENSURE THAT
ALL COMPONENTS ARE MARKED AS REQUIRED.  DISCONTINUE PAINING THE
HOOK FOR CORROSION CONTROL AS ANY CRACKS OR DEFORMITIES MAY BE
MASKED.  ENSURE THAT THE CHAIN CONNECTED TO THE RADIATOR LIFTING
DEVICE IS DESIGNED AND RATED FOR THE PURPOSE.
