a) Shop area: The Cincinnati full revolution mechanical power press being
used as a press
brake for bending metal parts was equipped with an unguarded foot pedal.
The opening of
the dies was greater then 3 inches, on or about July 7, 2006.
ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
One feasible and useful method to correct this hazard is to guard the foot
pedal to prevent the
inadvertent activation.
