place of employment which were free from recognized hazards that were
causing or likely to
cause death or serious physical harm to his employees in that, the
employer did not ensure
that each forklift was equipped with seatbelts to restrain the operator in
the event of an
accident and/or rollover:
(a) 1702 West Roosevelt Street, Phoenix, AZ 85007: A Hyster 550 forklift,
model
and serial numbers unknown, was not equipped with a seatbelt to restrain
the operator
in the event of a rollover or accident.
