in a
conspicuous location in the lift area; operators were not instructed in
the safe use and
operation of the lift; a periodic inspection procedure had not been
established for the lift;
the lift was not maintained and cared for to ensure its continued safe
operation; and the
lift was not permanently marked to show the manufacturer's name, rated
load
capacity,
model, and serial number.
