employee, conditions of
employment and a place of employment free from recognized hazards which
caused or were
likely to cause death or serious injury to employees:
An employee was observed performing miscellaneous roofing work, out of a
Snorkel
type aerial lift approximately 15' above grade, without the use of a
personal fall arrest
system or a fall restraint system.
