employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to injury while standing in bucket of front end loader:
(a)Lockhart Complex, Building 17, River Road, McKees Rocks, PA:  On or
about 8-11-04
two employees were injured when the bucket of the Volvo L50D, product id
code L50CP11844
in which they were standing and was lifted approximately 8-10 ft. above
the ground lost
hydraulic pressure and tilted down dropping the employees to the ground.
(b)Lockhart Complex, Building 17, River Road, McKees Rocks, PA:  On or
about
8-11-04
operators were permitted to operate the Volvo L50D, product id code
L50CP11844 even though
the machine was leaking hydraulic fluid.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is:
1.To prohibit employees riding or standing in the bucket of the front end
loader.
2.That the employer follow the manufacturer's recommended maintenance
program
for the Volvo L50D
