Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: a)Keeper Employee was standing on plywood which was lifted by a forklift at a height of approximately 26 feet to the ground below. Violation observed on or about 11/12/05. ABATEMENT NOTE:AMONG OTHER MEANS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD WOULD BE FOR THE COMPANY TO INSTRUCT AND ENFORCE THE PROPER USE OF EQUIPMENT TO LIFT PERSONNEL.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being crushed by the forklift truck in the event of a rollover: a)Liberty Village, Union Station, Union:Employee not wearing a seatbelt while operating the forklift. The work was being done outside on uneven ground, which presented a tipping hazard. Violation observed on or about 11/12/05. ABATEMENT NOTE:AMONG OTHER MEANS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD WOULD BE FOR THE COMPANY TO INSTRUCT AND ENFORCE THE USE OF SEATBELTS.
