occupied by employees:
(a)Line 1 Torit Dust Collector and Line 2 Scientific Collection Dust
Collector - Deflagration
venting was not provided to minimize the pressures developed in the event
of fire or explosion
within the dust collectors, on or about June 7, 2006.
(b)Line 1 Torit Dust Collector and Line 2 Scientific Collection Dust
Collector - Isolation devices
such as back-draft dampers were not utilized to prevent deflagrations
(fire/explosion) from
traveling through duct-work to charging and mixing stations occupied by
employees,
on or
about June 7, 2006.
(c)Line 2 Scientific Collection Dust Collector - Exhaust air was not
discharged to a restricted area
in that the discharge to the Line 2 Dust collector was located adjacent to
a designated outside
break area for employees, on or about June 7, 2006.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable
abatement method to
correct these hazards is to comply with the National Fire Protection
Agency (NFPA) 654, "Standard
for the Prevention of Fire and Dust Explosions from the Manufacturing,
Processing, and Handling of
Combustible Particulate Solids" (2006) as it applies to your operations
and equipment including but not
limited to:
Protect dust collectors in accordance with Section 7.13.1.3 or otherwise
adhere to the
exceptions as listed.
Provide isolation devices in accordance with Section 7.13.1.5 or otherwise
adhere
to the
exceptions as listed.
Discharge exhaust air to a safe area in accordance with Section 7.13.1.7
or otherwise adhere
to the exceptions as listed.
