Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing hazards:
(a) Location - Roof:
On or about 8/1/05, the rated load of a CM 1/2-ton under-hung crane was
not marked
on its' steel monorail.
Among other methods, one feasible and acceptable means to abate the hazard
is to have
a qualified individual calculate and install a load rating for the
crane/monorail in
accordance with ASME/ANSI B30.11.1980.

(a ) Location - Roof:
On or about 8/1/05, a CM 1/2-ton underhung crane was not inspected on a
periodic
basis.
Among other methods, one feasible and acceptable method to correct this
hazard is to
have a qualified individual inspect the crane and hoisting device at least
once in a twelve
(12) month period. Among the items to inspect include but are not limited
to:  deformed,
cracked or corroded members, loose nuts, bolts, rivets, worn or distorted
parts such as
pins, wheels, bearings, shafts, gears, rollers, locking and clamping
devices, bumpers,
interlock bolts and stops.  Excessive wear on brake system parts, linings,
pawls and
ratchets. Electrical apparatus for signs of pitting or any deterioration
of controller
contacts, limit switches and pushbutton stations. Inspect the crane in
accordance with
ASME/ANSI B30.11.1980.
(b) Location - Roof:
On or about 8/1/05, a CM 1/2-ton underhung crane was not inspected on a
frequent
basis.
Among other methods, one feasible and acceptable method to correct this
hazard is to
train employees to inspect the crane and hoisting devices at the beginning
of a workshift.
Among the items to inspect include but are not limited to:  all function
operating
mechanisms, deterioration or leakage in lines, drums, valves, drain pumps,
condition of
wire rope, brakes, upper limit switch, etc.  Inspect in accordance with
ASME/ANSI
B30.11.1980.
