Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by falling drill pipe sections: a) Steve Daugherty 2, Elk Twp, Clarion Cnty, PA - On or about June 18, 2008 the employer did not ensure that the U-bolt wire rope clips for the hoisting line (cat line) were installed in accordance with the manufacturers instructions (U-bolt contacts dead end and saddle contacts live end). One (1) wire rope clip was reversed with the u-bolt installed on the live end. ABATEMENT NOTE: Among other methods, feasible and acceptable method of abatement: 1) Follow the American Petroleum Institute (API) RP 9B, August 2002, Recommendation Practice on Application, Care, and Use of Wire Rope for Oilfield Service. 2) Follow the U-bolt manufacturers installation instructions

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by falling drill pipe sections and pulled into the rotating drum of the wire rope winch: a) Steve Daugherty 2, Elk Twp, Clarion Cnty, PA - On or about June 18, 2008 the employer did not ensure the pneumatic Tugger winch on the pipe trailer had a spring mechanism on the control lever to ensure neutral position when not in use. ABATEMENT NOTE: Reference American Petroleum Institute (API) RP 54 Section 6.8.8 Machinery and Toolsnery
