Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to severe burns and injuries from a dust fire:
(a)  On October 25, 2007, employees were exposed to a dust fire while
clearing a jam in the
cyclone collector.  The drying system, including the cyclone system, was
not properly operated,
evaluated and analyzed to protect employees and to prevent fire hazards.
Feasible means of abatement include, but are not limited to:
Comply with National Fire Protection Association (NFPA) 654, 2006 edition:
Standard
for the Prevention of Fire and Dust Explosions from the Manufacturing,
Processing, and
Handling of Combustible Particulate Solids; Chapter 4,"General
Requirements".
Conduct and implement a process hazard analysis of the facility, the
process
and the
associated fire and explosive hazards. Special attention must be directed
to section 4.6,
of chapter 4; which outlines both prescriptive and performance-based
methods of
minimizing fire and explosion hazards for this type of process.
Comply with National Fire Protection Association (NFPA) 654, 2006 edition:
Standard
for the Prevention of Fire and Dust Explosions from the Manufacturing,
Processing, and
Handling of Combustible Particulate Solids, Chapter 5,"Performance-Based
Design
Options". Management must retain or use the services of a qualified dust
explosion
expert to determine if additional measures are necessary to be
implemented, per this
standard, to provide for safe processing and handling of the combustible
paper pulp
material.  Of particular concern is an evaluation of the fire and
explosion potential for
the cyclone, the rotary dryers, and its associated duct work; and the
installation
of
fire/explosion protection methods where necessary as prescribed in chapter
7.
