Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Fall hazards created by the use of elevated work platforms which were not designed, constructed and/or used in accordance with the acceptable requirements of the American National Safety Standard for Rough Terrain Forklift Trucks, ASME B56.6-2002. The personnel platform and its use did not meet the use requirements of that standard in the following areas: a.)On 10/8/2008, a trained operator was not in position to control the rough terrain forklift or available to operate the controls. When the operator was not in the operating position, the truck wheels were not blocked to prevent horizontal travel. Among other methods, one feasible and acceptable abatement method to correct these hazards is to assure that the design, manufacture and use of all attachments for Rough Terrain Forklift Trucks are in accordance with the most current American National Standard, B56.6. No Abatement Certification or Abatement Documentation required.tion

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: The hazard of falling from the work platform while standing on the railing. Elevated work platforms were not used in accordance with the acceptable requirements: a.)On 10/8/2008, an welder was observed standing up on sections of the personnel platform's guard rails between 8 feet and 10 feet above ground level. The welder was wearing a harness and lanyard, but it was not anchored. b.)On 10/8/2008, the welder dismounted from the personnel platform by climbing up over the guard rails and down onto a ladder without any fall protection. Among other methods, one feasible and acceptable abatement method to correct these hazards is to assure that the design, manufacture and use of all attachments for Rough Terrain Forklift Trucks are in accordance with the most current American National Standard, B56.6-2002, Section 5.15; Elevating Personnel: 5.15.1(u): "Make certain that personnel maintain firm footing on the platform unless secured by body belt and lanyard." No Abatement Certification or Abatement Documentation required.tion
