employee, conditions of
employment and a place of employment free from recognized hazards which
caused or were
likely to cause death or serious injury to employees:
An employee was working from a JLG type aerial lift basket without the use
of personal
fall arrest system.  The employee was exposed to fall hazards of
approximately
18 to 20
feet above grade.
