The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to:
a) Getty Gas Station, Belmar, NJ. - An employee was exposed to
struck by hazards while working with an air/electric tire changer
that exploded causing the employee to become hospitalized. The
employer did not provide training to the employee.
b) Getty Gas Station, Belmar, NJ. - An employee was exposed to
struck by hazards while trying to mount a 16" tire onto a 16 1/2"
rim using the air/electric tire changer.

recognize hazards that would cause physical harm and/or death.

operators manual prior to using the Air/Electric Tire Changer,
Model EEWH311A, page 6, paragraph 1.

06/16/06, while operating the air/electric tire changer to change
a tire, the tire exploded producing hazardous flying debris and
sending the employee to the Jersey Shore Hospital.
