Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Getty Gas Station, Belmar, NJ. - An employee was exposed to struck by hazards while working with an air/electric tire changer that exploded causing the employee to become hospitalized. The employer did not provide training to the employee. b) Getty Gas Station, Belmar, NJ. - An employee was exposed to struck by hazards while trying to mount a 16" tire onto a 16 1/2" rim using the air/electric tire changer. 5(a)(1) Documentation 1a) Hazard to which employees were exposed - Struck by hazards. 1b) Hazard to which employees were exposed - Struck by hazards. 2a) Hazard recognition - The employee was not trained to recognize hazards that would cause physical harm and/or death. 2b) Hazard recognition - The employee did not read the operators manual prior to using the Air/Electric Tire Changer, Model EEWH311A, page 6, paragraph 1. 3a) Potential For Death Or Serious Physical Harm - On 06/16/06, while operating the air/electric tire changer to change a tire, the tire exploded producing hazardous flying debris and sending the employee to the Jersey Shore Hospital. 3b) Potential For Death Or Serious Physical Harm - On 06/16/06, while operating the air/electric tire changer to change a tire, the tire exploded producing hazardous flying debris and sending the employee to the Jersey shore Hospital. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD WOULD BE TO : 4a) Provide employees the proper training. 4b) Require employees to follow instructions of the operators manual.
