Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an asphixiation hazard: a.On or about June 5, 2006, two maintenance employees attempted to remove and replace two stainless steel 3-inch ANSI 150 Flange Gate Valves with 5/8-inch studs, 1 1/16 inch nuts,and metal gaskets on the south side of the Surge Tank next to the second working platform attached to the industrial ladder at 16.52 and 15.60 feet above the ground. The two maintenance employees did not use an appropriate portable hydrogen sulfide gas detection device to determine the atmospheric concentration of such gas in the flow pipes and the Surge Tank where potential releases could have occurred and created an immediatley dangerous to life or health(IDLH) atmosphere. Among other feasible means of abatement, provide and require the use of portable hydrogen sulfide detection instruments in accordance with the American Petroleum Institute Recommended Practices 55, titled Recommended Practices for Oil and Gas Producing and Gas Processing Plant Operations Involving Hydrogen Sulfide, section 6.3, which states; "If the atmospheric concentration of hydrogen sulfide could reach or exceed those levels stipulated in Par.6.1, portable hydrogen sulfide detection instruments (18) shall be available." ABATEMENT CERTIFICATION IS NOT REQUIRED.TION
