A) was observed to have the latch resting on the outside tip of the hook
making
the latch
inoperable.

10.1.2.1.2  Frequent Inspection:
10.1.2.1.2(c)(5):  Hooks shall be inspected for the following items:
damaged or malfunctioning latch (if
provided).
Among other methods,  one feasible and acceptable abatement method to
correct this hazard is to have an
adequate supply of safety latches available to be replaced by a competent
person  when damage occurs to latches
on existing hooks.
