employee, conditions of
employment and a place of employment free from recognized hazards which
caused or were
likely to cause death or serious injury to employees:
a)An employee working from a platform 27 feet in the air attached to a
forklift had
no means of fall protection.
b)Two employees were working on a 20-foot wide work platform that was six
feet
over the wheelbase of a rough terrain forklift.  The platform was 27 feet
in the
air.
