Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck by hazards during assembly and disassembly of a
gantry: On 07/25/07
an employee was fatally injured when the employer failed to ensure that
the materials and/or
equipment required to disassemble the Spanco "T" Series 3-way Adjustable
Gantry was used in
accordance with the manufacturer's instruction manual. Employees used a
come-along and a
chain fall wrapped around the I-beam and the cross brace of the "A" frame,
then removed the
securing pins from the legs causing the gantry to collapse, striking the
employee.  Among other
methods, feasible and acceptable methods of abatement include, but are not
limited to, the
following:  1) Develop and implement standard operating procedures for the
assembly, use, and
disassembly of the gantry. 2) Instruct all employees on the standard
operating procedures to
ensure they understand the safe method to assemble, use, and disassemble
the gantry. 3) Follow
the manufacturer's instruction manual when erecting or disassembling
(adjusting the height) of
the gantry.
