employee, conditions of
employment and a place of employment free from recognized hazards which
caused or were
likely to cause death or serious injury to employees:
An employee was working on a all-terrain forklift without a personal fall
arrest system,
which exposed the employee to potential fall hazards of approximately 21
feet.
