Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing and amputation from unintended activation:
(a)Fabrication Shop, 2197 Duss Avenue, Ambridge, PA:  On or about 8/27/07
the foot
treadle on the Pexton shear, Model S-3120B, Serial #K-5979, was not
guarded to prevent
unintended activation.  An employee suffered an amputation when the foot
treadle was
unintentionally activated and the employee's hand was in the point of
operation.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to guard the foot treadle.
