Title 40.1-51.1(a), Code of Virginia: Employer did not furnish employment,
and a place of employment which
were free of recognized hazards that were causing, or likely to cause
death or serious physical harm to
employees in that: Employee(s) was exposed to fall hazards while
performing job related duties when
employee(s) was permitted to work from a Sky Genie Variable descent
seatboard system without a full body
harness attached to independent line as required.
One feasible, and acceptable abatement method among others to correct this
hazard is to comply with the
manufacturers technical manual for the sky genie seatboard variable
descent system which reads: Independent
safety line must be attached to a SEPARATE anchor point meeting OSHA
standards capable of
supporting at least 5000 pounds. A rope grab shall be placed on this line
with no more than a three foot
lanyard attached to the D-ring in the center of the back of a full body
harness.
