Title 40.1-51.1(a), Code of Virginia: Employer did not furnish employment,
and a place of employment which
were free of recognized hazards that were causing, or likely to cause
death or serious physical harm to
employees in that: Employee(s) was exposed to fall hazards while
performing job related duties when
employee(s) was permitted to work from a job made box lifted by the forks
of a Ingersoll Rand VR-1056
forklift. The Ingersoll Rand VR-1056 forklift should not be used to
elevate personnel, as it is not designed for
this purpose.
One feasible, and acceptable abatement method among others to correct this
hazard is to comply with page 1-9
of the operators manual that reads:  DO NOT use you telescopic material
handler to lift people; it is not
designed for such use.
